PHOTO RELEASE CONSENT

I understand and agree that I may be photographed, or video or voice recorded while participating in the Program and Activities. On behalf of myself, I hereby grant permission to Hispanic Access to disseminate through broadcasting or publishing, pictures, and/or voice recordings taken during any Hispanic Access activities in which the student, parent, or volunteer appears, and consent to the use of such images and recordings for information, promotional, or publicity purposes. This Consent and Release of Liability, and any dispute arising hereunder or under the Program, shall be governed by the laws of the State of Massachusetts, excluding choice of law rules.  If any provision or part of this Consent and Release of Liability is held to invalid or unenforceable, such provision or part shall be severed and the remainder shall continue in full force and effect.

I have had enough time to review and seek explanation of the provisions contained above, have carefully read them, understand them fully, and agree to be bound by them.  After careful deliberation, I voluntarily give my consent and agree to this Consent and Release of Liability, on behalf of myself.

CONSENTIMIENTO PARA LA AUTORIZACIÓN DE FOTOS

Entiendo y acepto que puedo ser fotografiado, grabado en video o con voz mientras participe en el Programa y las Actividades. En mi nombre, autorizo ​​a Hispanic Access a difundir, mediante radiodifusión o publicación, fotografías o grabaciones de voz tomadas durante cualquier actividad de Hispanic Access en la que participe el estudiante, padre/madre o voluntario, y doy mi consentimiento para el uso de dichas imágenes y grabaciones con fines informativos, promocionales o publicitarios. Este Consentimiento y Exención de Responsabilidad, así como cualquier disputa que surja en virtud del presente o del Programa, se regirán por las leyes del Estado de Massachusetts, excluyendo las normas de elección de la ley aplicable.
